SHAKESPEARE CAMP POLICIES & PROCEDURES
Please read this information in its entirety. You are responsible for knowing the information it contains and are required to provide a printed copy with signatures before participating in camp.

CAMP REGISTRATION FEES
Camp Registration fees must be received in full by June 26, 2017 to be considered Registered in Camp.
Payment by Credit Card can be done at SBTS website –
shakespearebythesea.org/wp/kidscamp. Cost: $280 for 5 day camp, plus
performance.

Mail CHECK payable to: Shakespeare by the Sea CAMP, 777 Centre St, San Pedro, CA 90731

Checks Payments: All checks must be received and
processed prior to the first day of Camp. There will be a $50 fee for
all checks returned by the bank for nonpayment and possible removal of
student from Camp.

REFUNDS/WITHDRAWAL: Shakespeare by the Sea (SBTS)
has a no-refund policy. Withdrawal from the Camp program may occur only
due to family emergency and must be expressed prior to the first day of
the Camp program. Withdrawal from the Camp program after fees are paid may be subject to a $100 cancellation fee.
ATTENDANCE POLICY: Attendance at all scheduled Camp days is expected of each Camper.

1. Campers are allowed only 1 conflict that MUST be listed on the
registration form. Missing days not listed in advance may result in your
being removed from the Final Performance on July 7.

2. Special considerations may be granted for absences due to illness
or family emergency. In these instances, please call or text the Camp Director Whitney Cicero at (415) 298-5282 as promptly as possible. There are no refunds for missed days. There are no make up days.

3. ALL Campers must be signed IN and OUT of Camp each day by a parent or guardian.
Parents will be given instructions on where to park to pick up the
campers on the first day of camp. Campers must be picked up promptly at
1:30pm.

SHAKESPEARE BY THE SEA CAMP CODE OF CONDUCT

For all Camp Members:
1. Campers should wear attire appropriate for movement and dancing including close toed shoes. No flip flops please.

2. Return all costumes and props to SBTS staff at the close of each performance and rehearsal. Be careful with them.

3. Respect your fellow performers and staff. Be kind in your words and actions, remember everyone is doing their best.

4. Have fun!

For Parents:
1. Parents or legal guardians are responsible for camper transportation and pick-up.
2. Be on time. Leaving Camp early or arriving late may affect your
child’s inclusion in the Final Performance. Late pickups, more than 15
minutes after camp end time are subject to a $1 per minute charge.
3. If a parent would like to assist with snacks or costuming needs,
please let the Camp Directors know you are available to help.

For Parents & Camp Members:
DRUG & ALCOHOL POLICY: SBTS has a zero tolerance
policy regarding use of drugs and alcohol. Should SBTS Staff determine
there is a violation, disciplinary measures will be taken which could
result in permanent dismissal from Camp and exclusion from Final
Performance.

By signing below, you indicate that you have read and agree to the Shakespeare by the Sea Camp Policies and Code.

I have read and accept the Shakespeare Camp Policies and
Camp Code of Conduct. I have carefully read the conflict policy, have
included known conflicts on the form, will notify Staff of any new conflicts no later than the First Day of Camp July 3, 2017, and recognize the level of commitment required for this production.

Waiver, Release And Hold Harmless Agreement
In consideration of permission granted by SBTS theatre group allowing me
to participate in a SBTS Camp production (the “Activity”), I (together
with my parent or guardian, if I am under the age of eighteen or under a
legal disability) represent, covenant and agree, on behalf of myself
and my heirs, assigns, and any other person claiming by, under or
through me, as follows:

1. I acknowledge that participating in the Activity involves certain
risks (some of which I may not fully appreciate) and that injuries,
property damage or other harm could occur to me or others. I accept and
voluntarily incur all risks of any injuries, damages, or harm which
arise during or result from my participation in the Activity, regardless
of whether or not caused in whole or in part by the negligence or other
fault of SBTS, and/or its or their departments, trustees, affiliates,
staff, employees, officers, volunteers, agents or insurers (“Released
Parties”).

2. I waive all claims against any of the Released Parties for any
injuries, damages, losses or claims, whether known and unknown, which
arise during or result from my participation in the Activity, regardless
of whether or not caused in whole or part by the negligence or other
fault of any of the Released Parties. I release and forever discharge
the Released Parties from all such claims.

3. I agree to indemnify and hold the Released Parties harmless from
all losses, liabilities, damages, costs or expenses (including but not
limited to reasonable attorneys’ fees and other litigation costs and
expenses) incurred by any of the Released Parties as a result of any
claims or suits that I (or anyone claiming by, under or through me) may
bring against any of the Released Parties to recover any losses,
liabilities, costs, damages, or expenses which arise during or result
from my participation in the Activity, regardless of whether or not
caused in whole or part by the negligence or other fault of any of the
Released Parties.

4. I have carefully read and reviewed this Waiver, Release and Hold
Harmless Agreement. I understand it fully and I execute it voluntarily.

5. I (We) also understand and agree that SBTS will not be held responsible for injuries that occur to self or child while attending or participating in any SBTS function.

Authorization to Consent to Medical Treatment
In the event that a parent or guardian is unreachable, I (We) do hereby
authorize representatives of SBTS to serve as agents for the undersigned
to consent to any x-ray exam, anesthetic, medical or surgical diagnosis
of treatment and hospital care which is deemed advisable by and is to
be rendered under the general or specific supervision of any physician
or surgeon licensed under the provisions of the Medicine Practice Act on
the medical staff of any hospital licensed by the State of California
whether such diagnosis or treatment is rendered at the office of said
physician or at said hospital or some other site.

Photo and Video Release
I hereby give my consent to SBTS and its affiliates, assignees and
licensees to use my or my above-listed child(ren)’s name, voice,
likeness, verbal statements and portrait or picture (motion or still)
for advertising and/or publicity purposes or any lawful purpose
whatsoever in perpetuity, without compensation. Shall receive no
compensation of any kind for this release, now or in the future.
